Here are a few things you need to know to get off to the perfect start.

NEW PATIENT FORMS

Print and complete your New Patient Forms Packet to bring with you to your first appointment.

If you are unable to complete the forms ahead of time, please arrive 30 minutes early so you can complete them here.

 

APPOINTMENT CONFIRMATION

You will be receiving appointment reminder and confirmation texts and emails from our patient communications system, Smile Reminder (aka SolutionReach).

The automated schedule will remind you of your upcoming appointment 1 week in advance, and you will receive a message asking you to confirm 3 days in advance if you haven’t already done so.

You do have the option to let us know how you would like to receive appointment messages – either by email, text or both. We can even change the frequency if you’d like a little more of a heads up, or just a bit less contact. Just let us know and we’ll be happy to change it up for you.

We confirm every appointment via this system, so it will be important for you confirm each time you have an appointment so we know you will be here.

 

BEING ON TIME – OUR COMMITMENT TO YOU

Just like you, all of our patients value their time. We are committed to doing all that we can to be on time or ahead of time for each and every patient.

You play a vital role in helping us make that happen for everyone! We need your help with making sure your appointments are set on days and times when you have the least likelihood of delay or interruption in your schedule. Please make sure you arrive a few minutes ahead of your appointed time.

Our commitment to serve each of you is only as strong as the business it is built on. To maintain that integrity, our system will automatically charge a broken appointment fee of $50 for each cancellation within 48 hours.

We understand that sometimes unexpected things happen, and life takes over. We are happy to extend a one-time courtesy waiver for our patients who value and respect our time, as we do the same for them. For repeat instances and no call no show, we know you will understand if the need arises to implement this policy.
